When two penises grow independently during fetal gestation due to stress, chemical interference, or genetic abnormalities, it is thought to occur. Frequently, other genital and urinary abnormalities, including double kidneys, bladders, and scrotums, are present.

Diphallia is sometimes referred to as diphallus, diphallic terata, diphallasparatu, or penile duplication (PD). A common colloquial term for it is "double dick."
